Abstract Introduction: Preauricular fistulas are not uncommon congenital disorders in children. They are associated with imperfect auricle formation as a consequence of incomplete fusion of the auditory hillocks of the first and second branchial arches.

The sinus opening ( pit ) is usually located just in front of the upper part of the ear where the cartilage of the ear rim (helix) meets the facial skin.

Preauricular sinus is a frequent occurring congenital abnormality (circa 0.25% in the white race), but gives mostly no symptoms. In a certain amount of cases symptoms can arise as recurring purulent secretion or a preauricular abscess. If the disease is recognized an adequate therapy can be accomplished. Sometimes the disease is not recognized and diagnosed as a sebaceous cyst or a furunculus

Workup With regard to laboratory studies, culture samples may be obtained during drainage procedures. Imaging is not indicated for routine preauricular cysts and sinuses.
